Withdrawal Card
Please be advised, if you are on vacation, sick leave, etc. and you are on a Check-off System (employer deduction), it is your responsibility to see that all missed deductions are currently up to date.
GET A WITHDRAWAL CARD WHEN YOU LEAVE YOUR JOB
Be sure that you request a Withdrawal Card when being laid-off or terminating your employment. The fee for the withdrawal is $0.50, but all initiation fees and back dues must be paid in full before the card is issued.
It is the responsibility of the MEMBER to obtain a Withdrawal Card. Please take care of this matter as soon as possible. Call the office to verify the amount owed, so you will not be obligated to pay extra dues. Failure to request a Withdrawal Card may cause you to pay back dues.
