IMPORTANT INFORMATION
FOR PLAN MEMBERS WHO HAVE BEEN USING HEALTH CARE PROVIDERS OUTSIDE THE ANTHEM NETWORK
Effective May 17, 2004, you must use Anthem participating providers in order for services to be covered under this Plan. Charges made by non-network providers will not be covered.[1]
To locate an Anthem participating provider, please check the Anthem network provider directory or call Anthem to locate a network provider. Provider directories can be obtained by:
a) calling Anthem at 800-810-BLUE (2583),
b) visiting Anthem’s website:
providerdirectory.anthem.com or
c) calling the Plan office at 203-375-6088.

[1] Very limited exceptions to this rule are provided for Emergency Services and certain ancillary services, such as anesthesia or pathology, rendered while you are confined in an Anthem network hospital and you primarily are under the care of an Anthem network doctor.
